CLINICAL CARDIOLOGY: PHYSICIAN UPDATE Dietary Prescriptions to Control Dyslipidemias

Case No. 1 A 5-ft-8-in tall, 172-lb, 65-year-old woman has a total cholesterol level of 289 mg/dL; triglycerides, 108 mg/dL; LDL, 205 mg/dL; and HDL, 62 mg/dL. She does not smoke and has blood pressure of 122/70. There is no family history of premature coronary artery disease, but both her parents died in their 80s from massive strokes. Her older brother also had a stroke at age 60. She has tried 4 different statin drugs in the past 2 years and states that with each drug, she develops the insidious onset of lethargy, fatigue, and achiness, which resolve within 1 week of stopping the drug. She is highly motivated and is requesting dietary recommendations to reduce her LDL cholesterol.
